(Physician/MD qualifications required) Family Practice - Primary Care Physician

Compensation: $170K-$190K pr|yr
Job Type: Full Time, Monday - Friday. Day shift. Time split between 2 locations.
Our client a well established FQHC in Nassau County, NY is seeking a dedicated and skilled Primary Care Physician to add to there team.

Responsibilities:
Delivers high-quality medical care in alignment with professional standards and organizational policies.
Reviews patient medical histories during initial visits and maintains accurate documentation in the electronic medical record system.
Manages and coordinates medical care for assigned patients.
Evaluates walk-in patients, incoming calls, and referrals, ensuring appropriate medical care coordination.
Engages in regular clinical and administrative supervision.
Collaborates with the healthcare team to optimize patient care and coordination, participating in daily huddles and team meetings.
Supports patient care initiatives related to specialized programs, including care planning for high-risk patients.
Contributes to ongoing comprehensive care planning and review.
Provides onsite medical interventions as needed.
Facilitates patient referrals to in-network or external healthcare providers for necessary services.
Promotes cultural sensitivity in patient care and fosters awareness among staff.
Carries out tasks and initiatives as assigned by leadership.
Ensures timely follow-up on abnormal test results or physical findings.

Job Requirements:
Holds a Medical Degree (MD) from an accredited school
Currently licensed and registered as a physician with New York State
Board Certified or Board Eligible in a recognized medical specialty
Serving both school-age and adult populations
Ability to travel between 2 locations
A passion for working with individuals with intellectual and developmental disabilities
Medicaid and Medicare certified
