Job Description
Basic Function: To provide comprehensive social work services within a recuperative care setting, supporting individuals recovering from acute medical conditions who require transitional support. Responsibilities include assessing service needs, developing individualized care plans, implementing interventions through counseling, group work, referrals, and community partnerships, monitoring progress, and promoting health equity and self-sufficiency. This position operates at the full-performance level and requires minimal supervision.Examples of Work PerformedConduct intake interviews with clients recovering from acute medical conditions to assess their physical, emotional, and social service needs.Collaborate with clients families, medical providers, and community partners to develop and implement care plans focused on recovery and stability.Facilitate discharge planning from hospitals and ensure smooth transitions into the recuperative care facility and thereafter.Maintain accurate records in HMIS (Homeless Management Information System) and other databases to document client progress and service provision.Address immediate safety and housing concerns, intervening when clients are at risk of harm, and ensure safe, equitable environments.Coordinate access to community resources, including housing, employment, and financial assistance, tailored to client recovery goals.Provide direct therapeutic interventions, including individual and group counseling, to support mental and emotional well-being.Monitor client progress toward established care objectives, adjusting plans as necessary to meet evolving needs.Transport clients or arrange transportation to medical appointments, court hearings, or other necessary services.Engage in crisis intervention to address urgent needs such as housing, food, clothing, or safety.Evaluate foster or transitional care providers for potential placements, as applicable.Develop and facilitate training for team members and community partners on best practices in recuperative care and trauma-informed services.Represent Care Chexx in community committees and program planning efforts, advocating for comprehensive care solutions for vulnerable populations.Provide supervision or guidance to interns, volunteers, or junior staff as needed.Prepare reports and documentation for court proceedings, case reviews, and service referrals.Consult with healthcare professionals, legal advisors, and other stakeholders to ensure coordinated service delivery.Monitor compliance with program guidelines, court orders, and service expectations, reporting progress and barriers as needed.(The work assigned to a position in this classification may not include all possible tasks and does not limit the assignment of additional responsibilities. Regular attendance according to the positions management-approved schedule is required.)Minimum Qualifications Education: Bachelors degree in a social/behavioral science or equivalent. Masters Degree or any combination of 23 additional credits or 230 continuing education hours in fields related to social work or mental health for higher level or specialty level appointment.Experience: None for entry level appointment. Five years of experience in social work or a related field. Experience in medical respite, health services, or supportive housing is preferred for higher appointment. Supervisory/Managerial Responsibility: May provide work direction to interns, volunteers, or junior staff.Work Environment: Work occurs in a variety of settings, including recuperative care facilities, hospitals, and partner agencies. Frequent travel may involve exposure to inclement weather, unsanitary or hazardous environments, and communicable illnesses. The role involves managing challenging situations, including interacting with individuals in crisis or exhibiting hostile or aggressive behavior.Direct contact with individuals experiencing homelessness, including those with infectious or other complex health conditions. Care Chexx personnel must complete a baseline individual TB risk assessment, symptom screening, and TB testing (e.g., TB skin test or TB blood test) upon hire/pre-placement.Care Chexx workforce members are recommended to be fully vaccinated and boosted against COVID-19.Care Chexx workforce members are recommended to be fully vaccinated against Hepatitis B upon hire with (2 dose series of Heplisav-B with doses separated by at least 30 days). Internal/External Relationships:Daily contact with clients to assess needs, develop recovery plans, and monitor progress.Collaboration with medical professionals, mental health practitioners, housing providers, and community organizations to ensure comprehensive care.Engagement with legal entities (e.g., courts, attorneys) to support clients in navigating legal processes related to recovery and stability.Regular interaction with Care Chexx leadership, public health staff, and community partners to align services with organizational and client needs.Representation of Care Chexx in community forums and committees to advocate for effective recuperative care solutions.
